CSB Probe Into Fatal Mill Explosion Highlights Gas Detection Failures
Safety+Health Magazine reports that the Chemical Safety Board is emphasizing gas monitoring equipment as it continues investigating a fatal hydrogen sulfide release at the Woodland Pulp mill in Baileyville, Maine. Two workers died in January after highly toxic hydrogen sulfide escaped through holes and process openings during a partial mill shutdown. Investigators found operators had drained equipment and routed process fluid into an acid sewer pipe as part of a natural gas management protocol. When machinery removing hydrogen sulfide from that pipe was shut down, toxic gas spread through the facility. The workers, who were completing a task unrelated to the shutdown, collapsed and were not found until hours after emergency crews had cleared the scene. CSB member Sylvia Johnson said Woodland Pulp knew hydrogen sulfide could form in the acid sewer piping but lacked adequate systems to monitor or mitigate the hazard. The agencyโs final report, with complete findings and recommendations, is still pending.
