The U.S. Chemical Safety and Hazard Investigation Board has released its final report on the August 11, 2025, explosion at U.S. Steelโ€™s Clairton Coke Works in Pennsylvania, ISHN reports, citing an informal maintenance procedure and inadequate building siting as key causes of a blast that killed two workers and injured 11 others.

Market Impact

The explosion occurred when U.S. Steel employees and contractors from MPW Industrial Services attempted to exercise a cast iron double disc gate isolation valve on coke oven gas piping feeding Battery 13. Workers used a pump truck to apply pressurized water to the valve seats, a practice U.S. Steel had used informally for at least three years without a formal written procedure. The water became trapped between the valveโ€™s two gates, pressure built beyond the valveโ€™s capacity, and the 1953-manufactured, 2013-refurbished valve failed catastrophically, releasing toxic flammable coke oven gas that ignited within minutes. Property damage is estimated at $52.5 million.

CSB Chairperson Steve Owens said the incident โ€œwas the result of an ad hoc informal procedure, poor facility siting, and an ineffective process safety management system.โ€ Investigators noted that occupied buildings, including control rooms and a break room, sat less than 20 feet directly above the gas piping and were not designed to withstand an explosion. CSB Investigator in Charge Drew Sahli said locating those buildings elsewhere โ€œcould have been far less severe.โ€ The CSB has recommended U.S. Steel conduct a siting evaluation of all occupied buildings at the facility, develop a written water-washing procedure for valves, and build a comprehensive process safety management system for coke oven gas operations. Parent company Nippon Steel North America was told to establish corporate-level process safety governance and auditing, while MPW was directed to create written procedures and training for cleaning piping systems carrying flammable or toxic gas.

  • Any contractor performing valve exercising, cleaning, or maintenance on gas-handling piping should confirm a written procedure exists before using water, steam, or pressurized fluid on isolation valves, especially where trapped-volume conditions can form between double gates or seats.

  • Industrial cleaning and pump-truck service providers like MPW should expect new customer-side training and documentation requirements for jobs involving pressurized water near piping systems carrying flammable or toxic gas, based on the CSBโ€™s direct recommendation to MPW.
  • Mechanical, E&I, and maintenance subcontractors on steel, coke, and chemical processing sites should ask clients whether a facility siting evaluation for occupied buildings near hazardous piping has been completed, since CSB specifically recommended U.S. Steel conduct one for all buildings at Clairton.
  • Companies bidding coke oven, blast furnace, or similar gas-process work should review whether ad hoc field practices, like the undocumented water-washing method used for at least three years at Clairton, have crept into their own crewsโ€™ routines and require formal sign-off before continuing.
